Established patients may use the portal to request medication refills, request appointments, and update demographic and contact information.
We typically activate the patient portal at your visit at your request. If you are an established patient and are in need of a username and password for the portal, please call our office at 828-253-2533 and request this by providing your email address. Once we receive your request, we will send you a link to login which must be used within 24 hours.
If you are an established patient and do not have access to our portal and would like to request that your medical records be forwarded to another medical office, please print, complete, and sign the following form and we will forward your records upon receipt. You may send the form via mail or fax to our office at 828-253-2536.
